FIRST in Texas requires all participants, coaches and volunteers to complete our Consent & Release Form before attending / competing at any of our event. This is a requirement of our insurance carrier and reminds us all about the importance of safety.

Q: Why do I submit this online?
A: We ask that you do this online to have a permanent record of your form and to help safeguard your personal information from traveling from events to our secure storage.
Q: Do I submit a form for all volunteer roles?
A: We just need one form a year (season). If you participate as a team member and then decide to volunteer… one form is fine.
Q: Which form do I chose?
A: If you are submitting a form for a participant (child) click the form for a child. If you are submitting the form as an adult (coach / mentor / volunteer), please select the adult form.
